Baptismal Record Information Form
Please complete the form below no later than January 25, 2023. If your child was not baptized at St. Vincent's and does NOT attend St. Vincent School, the Family Faith Formation Office requests a photocopy, FAX, or picture of your child's baptismal record. You may email to debbieblackburn@saintv.org, mail, drop off in the Church Office, or FAX a copy of the baptismal certificate to the Church Office (260-497-9405) as soon as possible. Sacraments cannot be conferred without proof of baptism.
