Catholic Kidz Camp is currently full, however, additional spots are opening daily! You may complete this registration form and your child will be placed on a waitlist.  I will contact you when spots become available.  I will do my best to make sure everyone can attend!

I grant permission for my child/children named above to participate in the St. Vincent de Paul Catholic Kidz Camp held June 6 - 10 from 9:00 AM to Noon.

I will not hold the St. Vincent de Paul Parish, or St. Vincent de Paul Parish staff, volunteers, or chaperones associated with this gathering responsible in the event of injury. Further, I agree to accept any and all financial responsibility as a result of scheduling necessary emergency medical treatment.

I will not hold St. Vincent de Paul (Parish / School) responsible for any injury incurred on the trip to and from this gathering. I hereby warrant that to the best of my knowledge, my child is in good health, and I assume all responsibility for the health of my child.

In the event of emergency, I hereby give permission to transport my child to a hospital for emergency medical or surgical treatment. I wish to be advised prior to treatment by the hospital or doctor.
